For individuals returning from incarceration, our Reentry Support Coordination focuses on the first critical weeks: safe housing connections, ID and paperwork, job readiness referrals, and clear guidance on staying compliant with supervision. Older adults benefit from focused Elderly Care Planning, where we look at safety, medical appointments, meal access, and caregiver needs so you can stay independent as long as possible.

We also roll up our sleeves with Benefit Application Assistance, sitting beside you to complete and submit forms for SSI, SSDI, Medicaid, pensions, and other programs. No more guessing what a question means or missing deadlines because the paperwork felt overwhelming.

To keep everything organized, we use Individualized Care Planning tools: easy-to-follow worksheets, checklists, and guides that help you see your goals, appointments, and supports in one place. Our Resource Referral Services add a printed, easy-to-read pack of trusted contacts and instructions for local clinics, food programs, housing resources, and legal or employment supports. Life is not a straight line, so we stay with you when things get tough. Our Crisis Intervention Services offer immediate support when you face a sudden loss, conflict, or mental health concern, helping you stabilize and connect to the right help quickly. With our Progress Monitoring journal, you track wins - big and small - so you can see how far you have come and what the next step looks like.
